Filling Out the Service Order Form

How to fill out the Service Order Form:

1. Download the Service Order Form.
2. Print the form on paper.
3. Fill the form out (download the sample [service_order_model.pdf - 135kB]). Please pay extra attention when describing the reason for return. If the calibrator requires repair please describe the fault as clearly as you can.
4. Pack the service order form and the calibrator into the same package.
5. Contact your local representative or Beamex directly and ask how to proceed and where to send the equipment.

Note!

Please state all your contact details clearly.

If you even suspect that the device might be contaminated, please name the chemical which might have caused the contamination.

Tick the box "delivery reserved for week number ___" only if you already have reserved a time for repair and/or recalibration with Beamex. Also mark which kind of calibration certificate you want. The official calibration certificate is valid in all EU countries.

When you describe the reason for return, please try to be as specific as possible.
